Introduction

• Short, hypermobile 1st metatarsal.

Pathology

• hypermobile 1st metatarsal is often inadequate for push-off

Clinical-manifestations

• characteristic weight-bearing pattern centered at the 2nd & 5th (rather than 1st & 5th) metatarsal heads
• knee & foot pain ( metatarsalgia) especially with activity

Management

• modification of footwear to include an inner border of the shoe bottom that provides a more stable base for push-off may alleviate symptoms
• foot orthoses
