  • Tradename: Neurontin.

Indications

Contraindications

Dosage

  • start 300 mg PO QHS
  • increase over a few days to 300-600 mg PO TID
  • max 3.6 g/day
  • therapeutic doses > 900 mg/day
  • Pediatrics: Start 15 mg/kg/day, max 45 mg/kg/day.
  • Tabs: 100, 300, 400 mg. Dose adjustment in renal failure:

* creatinine clearance          dose 
*   > 60 mL/min                400 mg TID 
*  30-60 mL/min                300 mg BID 
*  15-30 mL/min                300 mg QD 
*   < 15 mL/min                300 mg QOD


  • 200-300 mg following each 4 hours of hemodialysis
