Drug-interactions

• ACE inhibitors increase prostaglandins that cause vasodilation; NSAIDs inhibit cyclooxygenase, thus formation of prostaglandins
• prostaglandins act on afferent glomerular arterioles, ACE inhibitors act on efferent glomerular arterioles
• increased risk of hyperkalemia
• NSAIDs may interfere with antihypertensive effects of ACE inhibitors
• when combined with diuretic, further increases risk of acute renal failure [2]

Management

• 81 mg of aspirin recommended for use in combination with ACE inhibitor, when both are indicated.
